The Five Essential Conditions
Policies that aim to ensure availability, accessibility, affordability and quality of preventative and health care services and interventions. For example, health protection, health promotion and improvement, primary, secondary and scheduled care.
Policies that aim to provide economic security and support to reduce the health and social consequences of poverty and low income throughout a person’s life. For example, financial support for parents, older people or unemployed.
Policies that aim to ensure opportunities for, and access and exposure to living conditions and environments that have a positive influence on people’s health and well-being. For example, planning, good quality and secure housing, clean air, green spaces.
Policies that aim to develop and strengthen social relations and community assets, including education, skills, community resources and meaningful social interactions to promote learning, and protect and promote health and well-being throughout a person’s life. For example, improving training, apprenticeship, building community cohesion and resilience, trust, sense of belonging
Policies that aim to improve the health impact of employment, working conditions and workplace equality. For example, availability of work, a living wage, physical and mental demands, ensuring health and safety at work.
The Welsh Health Equity Solutions Platform is a gateway to data, evidence, health economics and modelling, policies, good practice, innovative tools and practical solutions to help improve population wellbeing and reduce the health equity gap in Wales and beyond. It is structured around an innovative WHO framework of ‘Five Essential Conditions’ for healthy prosperous lives for all. The Platform will be link with and feed into a WHO health equity gateway, providing an example and inspiration for countries to learn and follow, as well as to contribute and share.
The WHESRi and the Solutions Platform have been developed and are progressing in close collaboration with relevant professionals and experts across Welsh Government, the WHO, academic bodies and other key stakeholders and partners across Wales and internationally. It is being informed and shaped by continuous stakeholder and user feedback and emerging evidence, and reflecting dynamic national and global priorities.
Working with stakeholders and users, the Solutions Platform will be updated and expanded to include projects and case studies focused on health equity from Wales and beyond. Get in touch to get involved.
The Solutions Platform contains:
- Data Explorer: Select and display indicators filtered by essential condition, with the option to download for use in presentations
- Health economics and modelling: Explore the interactive ‘Cost of health inequalities in Wales’ database and view health economics resources
- Policy: Explore Welsh policies and legislation that help to reduce inequity and create a more sustainable and fairer society in Wales
- UK and international: View resources from the UK and around the world, presenting knowledge and insights on health equity
- Vulnerability and unmet needs: View resources exploring health equity, vulnerability and unmet needs in Wales
- Solutions: View resources on potential solutions to help address health equity
- Generate report: Generate and download customised reports presenting selected information from across the Solutions Platform filtered by information type (e.g. data, UK and international, solutions) and essential condition

The Five Essential Conditions
The Welsh Health Equity Solutions Platform is organised around an innovative WHO health equity framework of ‘five essential conditions’ [p.84] developed by the WHO European Office for Investment for Health and Development in Venice, Italy. The five essential conditions are the multi-sectoral policy action areas to address complex and persistent health gaps and increase opportunities to live healthy and prosperous lives for all.
Health and health services

Health and income and social protection
Policies that aim to provide economic security and support to reduce the health and social consequences of poverty and low income throughout a person’s life. For example, financial support for parents, older people or unemployed.
Health and living conditions
Policies that aim to ensure opportunities for, and access and exposure to living conditions and environments that have a positive influence on people’s health and well-being. For example, planning, good quality and secure housing, clean air, green spaces.
Health and social and human capital
Policies that aim to develop and strengthen social relations and community assets, including education, skills, community resources and meaningful social interactions to promote learning, and protect and promote health and well-being throughout a person’s life. For example, improving training, apprenticeship, building community cohesion and resilience, trust, sense of belonging.
Health and employment and working conditions
Policies that aim to improve the health impact of employment, working conditions and workplace equality. For example, availability of work, a living wage, physical and

The Welsh Health Equity Solutions Platform has been developed, and is being further updated and expanded, in close collaboration with relevant colleagues and experts across Public Health Wales, the Welsh Government, the World Health Organization, and other key stakeholders and partners across Wales and internationally.
